Survivorship Program
As a cancer survivor, you may experience medical and emotional challenges after the completion of treatment. To help guide you through these challenges and keep you on your current path of health and wellness, Summit Health Cancer Center offers our breast cancer patients a comprehensive Survivorship Program.
What is our Survivorship Program?
Survivorship care support is a critical component of follow-up cancer care. Our Survivorship Program is led by our Advanced Practice Nurses and Physician Assistant in coordination with an Oncology Patient Navigator. At your Survivorship Program visit, these providers will review potential long-term treatment side effects, set you on course for proper lifestyle management with diet and exercise recommendations, and help you talk through any feelings and emotions you have regarding your cancer journey. Additionally, they will review your treatment summary and survivorship care plan and make further recommendations for cancer prevention and screenings. Our Oncology Patient Navigator will review resources with you, check on your progress and address any follow-up questions.

A Survivorship Program visit is covered by most insurance. These visits are not intended to take the place of primary care follow up or routine health visits with your oncology provider, and all recommendations should be followed in conjunctions with guidance from your doctor.
