Our Location
We are located on the second floor of the Lawrence Pavilion. For your convenience, please park in the lots nearest the Lawrence Pavilion entrance.
Before leaving home, please be sure you have your:
- Insurance cards
- Referral ( if you have or need one)
- Completed Health History Form
- Copies of all previous hearing, balance, and test results related to your hearing or balance, which we will keep with your files
- Images from all MRI and CT scans of the ear and brain if you have had them taken at another practice (you may take your MRI and CT scans home with you after your visit)

Health Insurance
Summit Health accepts insurance from many companies. If you need ear surgery and have out-of-network benefits, please call our office at 908-277-8681 to discuss coverage options. In many cases, we can negotiate with your insurance company to provide coverage for your surgery.
