Jade Reyes, DNP, APN, FNP-BC (formerly Wells Porrmann) is a member of Summit Health’s Family Medicine team. Dr. Reyes is a nurse practitioner with specialty certification in family medicine. Dr. Reyes holds a Doctor of Nursing Practice degree (DNP), the highest level of educational training in nursing. Prior to becoming a nurse practitioner, Dr. Reyes worked as an acute care nurse in the medical/surgical/renal and pancreas transplant step-down unit, as well as in the operating room. Dr. Reyes also has nursing leadership experience, most recently with Summit Health’s Ear, Nose, and Throat (ENT) and Allergy team.

Dr. Reyes provides a comprehensive approach to care—addressing patient’s physical, emotional, and social needs. She emphasizes treating patients as if they were family, which lends itself to what she says she enjoys most about her work: “the ability to form long-lasting relationships with patients across their lifespan.”

Dr. Reyes has participated in various research studies published in peer-reviewed journals, and she is a Sigma Theta Tau International Honor Society of Nursing inductee.

When she’s not seeing patients, Dr. Reyes enjoys spending time with her husband and rescue dog. She also enjoys traveling, crafting, and volunteering at local dog shelters.
