Helpful tips for new patients


How should I prepare for my initial appointment?

  • In the days leading up to your appointment, you will electronically receive Dr. Sinha's Intake Form via text. This form is a pre-requisite to the appointment. Please be sure to fill out all fields as this will help us get the most out of the visit.  
  • It is often helpful to bring a written list of all concerns and questions with you, if you cannot enter all of them on the intake form. 
  • Please confirm which location your appointment will be at (Florham Park or New Providence) and arrive 10-15 minutes prior to appointment time. 

What should I expect?

  • During your first visit, we will ask you about specific concerns you have about your child's neurological health. You can expect that we'll have an open conversation about their symptoms.  We will then review your child's medical/developmental history and the intake form, after which a neurological examination will be conducted (mental status, eye/facial movements, strength, sensation, reflexes, gait, coordination). There will not be any shots or swabs!  Then we'll talk about diagnosis and next steps. Diagnostic tests (i.e. EEG, MRI, CT) are not done during the consultation visit but will be arranged if deemed necessary.

Note: The patient must be present at every appointment. I recommend speaking openly with your child about the reason for the visit. Please utilize the portal for any sensitive information that you would like to relay to Dr. Sinha.

  • We do not conduct any formal neuropsychological testing.  If this is what you are looking for, please call your insurance company for a list of psychologists who provide this testing. There is no specific diagnostic test for ADD/ADHD.*

*With regard to dyslexia evaluations, we do not conduct objective testing. However, Dr. Sinha can perform a screening based on clinical history and assessment of signs/symptoms. If further testing is needed, referrals will be provided.

What should I bring?

If your child has had any prior neurological or general medical evaluations, such as imaging, blood work, genetic testing, consultations, etc., please be sure to bring all \the records with you. It is also important that you bring any school evaluations (e.g., teacher/therapist feedback, psychological/educational evaluations, physical/occupational/speech therapy evaluations/reports) to the visit. If your child has an IEP or a 504 plan, please bring a copy of that as well. 

How do I send records or information to your office ahead of time?

Please fax documents to 862-465-9914 or you may upload documents through your patient portal.

If you do not have patient portal access, please click here to register. If you do have patient portal access, use the same link to log in. In order to upload documents and send them to us through the patient portal, on the home page click on "send a message", which is in the middle of the page. Then choose Dr. Sinha as your provider. At the bottom there is a tab to attach files. You may also write any pertinent notes within the message.

  • Please request refills when you are approximately a week away from running out of medication. Do not wait until the last minute.  
  • Please do not use the after-hours line for medication refills; requests should be made during business hours only from 830am-5pm. 
  • When you receive a text from our office to complete a form, please be sure to submit it before your scheduled appointment. 
  • You may use the portal to send questions. Portal messages are reviewed within 48 hours. Please do not send urgent messages through the portal. 
  • Forms/Reports/Letters have a turnaround time of 5-10 business days.  
  • Your child must be present for the visit, including virtual appointments. 
  • Please be on time for your appointment. If you arrive late, the appointment will likely be rescheduled. If we are able to accommodate you, the appointment will be shortened.   
  • If you would like to speak with Dr. Sinha privately for a portion of the visit, you must bring another adult to supervise your child in a separate area.  

Helpful tips for established patients


How do I retrieve documents or letters sent from the office to my patient portal?

To access letters or documents from your child's portal, log in from a desktop computer or laptop. Documents will not be viewable from a cell phone. Once logged in, click on “My Health” and then “Medical Forms”. You will see "Signed documents from your providers”.

For follow-up appointments:

You will receive a text message prior to your child's follow-up appointment. We will be asking for updates on symptoms, medications, therapies, etc. Please make your best effort to respond to our form via text message. Thank you.
